Please note: Initial patient cards are $25.00 (Inc gst). Payment is required in advance. Once you’ve completed the form, we will be in touch with an invoice for your payment. You will also have an option to select Delivery or pickup. Delivery is $7.50 nationwide.

Replacement cards will be invoiced at $15.00.
